What Is Transferrin?

Transferrin is the protein that carries iron through the bloodstream, picking it up and delivering it to the tissues that need it. Because it is the main transport vehicle for iron, clinicians measure transferrin as part of the iron studies they use to help evaluate how the body moves iron.

What transferrin is

Iron does not travel freely in the blood. Instead, it is bound to a carrier protein made mostly by the liver, and that carrier is transferrin. Each transferrin protein can hold iron and release it where it is needed, acting like a shuttle that moves iron from where it is absorbed or stored to the tissues that use it, especially the bone marrow that makes red blood cells.

A transferrin measurement reflects how much of this transport protein is present in the blood. Because transferrin and the body's iron handling are linked, the amount of transferrin can shift as the body adjusts to how much iron it has available, which is part of why clinicians find it informative.

Clinicians order transferrin when they want to understand the transport side of the iron system — not how much iron is stored or circulating, but the capacity that carries it.
