This notice covers Celio Health and the personnel and partner organizations involved in fulfilling your lab order: the independent physician network that reviews and authorizes test requests, and the reference laboratories that collect and analyze your specimens.
Treatment. We share your information with the ordering physician who authorizes your test and with the laboratory performing it, and we return results and explanations to you.
Payment. We use your information to process the payment you make at checkout. Celio Health is cash-pay; we do not bill insurers.
Health care operations. We use your information to run our service: quality checks, customer support, and secure record-keeping.
Required by law. We may disclose information when required to do so, such as mandatory public-health reporting of certain lab results or a valid legal order.
Other uses and disclosures, including marketing and any sale of your information, will be made only with your written authorization, which you may revoke at any time. We do not sell your personal or health information.
We are required by law to maintain the privacy and security of your protected health information, to notify you promptly if a breach occurs that may have compromised it, to follow the duties and practices described in this notice, and to give you a copy of it.
If you believe your privacy rights have been violated, contact our privacy team at privacy@celiohealth.com. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
